Referral Instructions (Kindly advise the referring veterinarian to convey points 2 and 3 to the owner when making the appointment to avoid affecting their rights. Thank you!)
1. After completing this form, please take a photo or scan the referral form and email it to hellopeter.vet@gmail.com. We will respond and schedule the appointment within two business days. Please be patient. If the owner does not receive a notification within two business days, please call (04) 2472-8828 or message through our Facebook page (https://www.facebook.com/vet.dentistry.oral.medicine/ or click on the Facebook icon link in the upper right corner of the hospital's official website).
2.For detailed examination reports (such as blood test reports, pathology reports, or images from diagnostic imaging), please attach them in email format. Bring imaging data (such as ultrasound, X-rays) in the form of a disc during the appointment for uploading to our imaging system.
3.To avoid affecting other patients and owner rights, if the owner needs to cancel or reschedule a scheduled appointment or surgery, please inform the hospital by calling or messaging through the Facebook page one business day before the appointment or surgery day before 9 PM (excluding national holidays and days off). Failure to notify in advance and not showing up will be considered a no-show. If there are two consecutive no-shows, we will no longer accept appointments for that pet owner.
4.Patients who have not returned for over a year are considered new patients and must make a prior appointment for dental consultation. Same-day surgery cannot be arranged.
